Our healthcare system continues to navigate unprecedented times, coping with clinical and support staffing shortages and the closure of healthcare providers leading to increased demand, especially on those of us still providing community-based care. We have continued to pivot in the face of these challenges while providing world-class care, research, education, and services.
During fiscal year 2023, we provided clinical care for more than 28,000 patients who made than 120,000 visits, including 5,000 trans and gender diverse (TGD) patients and nearly 1,800 people living with HIV. We continued to push back against discriminatory laws being introduced across the country that would curb LGBTQIA+ rights, especially the rights of TGD young people. We published Transgender and Gender Diverse Health Care: The Fenway Guide, the first case-based textbook to address the comprehensive health care needs of TGD adults.
At the same time, we published a paper in LGBT Health reporting on the impact of a four-week elective course at Harvard Medical School that prepares students to become physician-leaders in LGBTQIA+ health care. The course is one of the few available to medical students that provides opportunities for direct care experiences with patients. We launched the Harvard Medical School-Fenway Health LGBTQIA+ Health Fellowship and the Kenneth H. Mayer Fellowship, both designed to train future leaders in LGBTQIA+ and HIV research. And we piloted the rollout of injectable PrEP, which is now widely available to our patients who use PrEP for HIV prevention.

Fenway Health
In 2020, Fenway Health committed to becoming an anti-racist organization by working to overcome the racial inequities that exist in the services we provide, the people we reach, and the outcomes we achieve.
The Fenway Health Board of Directors unanimously approved an ambitious, 5-year strategic plan in June 2021 to guide the work of the organization over the next five years. The plan centers a Racial Action Equity Plan (REAP) designed by a team of employees from across Fenway and several board members to ensure that racial equity is being addressed in eight key areas of focus in the organization.
We completed the first of three phases of the plan in June 2022. As part of the plan and in preparation for the next phase of our efforts, we devoted concentrated time to reflect on our progress, evaluate our successes and setbacks, and realign our priorities.
We launched Phase Two of our efforts in January 2023. In the spirit of accessibility, transparency, and accountability, we are pleased to share what we have learned, and where we are going.
We engaged multiple internal and external stakeholders throughout the realignment period. We received significant, insightful feedback suggesting that we do more to amplify and address the distinctions between institutional goals, team-level goals, and individual goals toward antiracist practice.
The blueprint for Phase Two of our work incorporates this and other guidance we received. The plan identifies and brings forward those Phase One efforts in which the work continues. It sets forth a simplified structure for better understanding and accessibility, and it forecasts the capacity, bandwidth, and resources we need to deploy.
We are supporting teams in their efforts to create antiracism plans specific to their day-to-day work, as well as renewed/amplified effort to achieve institutional goals related to our Board of Directors, leveraging patient race data via our new EPIC medical records system to improve outcomes based on race, continued improvement of our investment in BIPOC business as part of our procurement practice, and creating new training and development opportunities for our staff. All this work is in service to improving and sustaining the care we provide our patients and clients.
We continue to be committed to making this plan accessible to our community and to ensuring accountability for its implementation. The plan identifies at least one of point of contact who bears final responsibility for moving an item forward, and it contains provisions for regular and transparent updates.

Fenway has helped pioneer a philosophy of integrated care that looks at each patient as a whole person and our staff works as a team to meet the needs of everyone who comes through our doors.
Patient-Centered Medical Care
Fenway Health is certified as a National Committee for Quality Assurance Level 3 Patient-Centered Medical Home (PCMH). The PCMH model of primary care emphasizes care coordination and communication to transform primary care into “what patients want it to be.”
Family Medicine
Fenway Health’s Family Medicine Program allows us to provide services to patients of every age group, from the youngest to the oldest.
Transgender Health
More than 5,000 transgender patients currently receive care at Fenway Health locations. The Transgender Health Program’s integrated model of care is one example of how we continuously work to lower the barriers trans and gender diverse people face in accessing care. From their first day at Fenway Health, our providers are trained to provide all aspects of care through an affirming, trauma-informed model. We also consider hormone therapy to be a part of primary care, and all of our Internal Medicine and Family Medicine providers are happy to work with our patients towards their individual goals.
Reproductive Health
Fenway Health’s Reproductive Health Equity Program seeks to meet the healthcare needs of all individuals using its services, across the spectrum of identity, including BIPOC, LGBQIA+, transgender, and gender diverse communities. We offer community engagement and outreach programming that encompasses reproductive health education and holistic health. Our obstetrical services care program partners with Beth Israel Lahey Health to provide prenatal care for patients in all stages of pregnancy. The program also offers comprehensive birth control and emergency contraception services.
Public Health Programs
AIDS Action is the public health division of Fenway Health and our services are designed for those most vulnerable to HIV/AIDS. In fiscal year 2023, our Access Drug User Health Program distributed 398,000 sterile syringes, collected 113,000 used syringes, and trained more than 340 people in how to use Narcan to reverse an overdose. Additionally, more than 14,500 HIV tests were administered by Fenway medical and public health staff and our community health workers offered health navigation and connection to care for clients and patients who needed them.
Insurance Enrollment
Under Fenway’s health insurance enrollment program, patients without insurance can be enrolled in coverage under MassHealth, Commonwealth Care and Health Safety Net.
Fenway Pharmacy
Approximately $1 million in free and discounted medication was given to patients in need by the Fenway pharmacy’s two branches. The pharmacies also offer a free home delivery service for homebound patients and others unable to prick up their medication. Together, the two locations fill well over 500 prescriptions per day.
Alternative Insemination
Approximately 850 babies have been conceived through Fenway’s Alternative Insemination (AI) Program since outcomes tracking began in 1984. The Fenway Health AI Program is believed to be the first of its kind in the United States. In fiscal year 2023, the AI program saw 15 new pregnancies. The AI Program held it’s first “AI Family Reunion” event in 2023, bringing together current and past families who have participated in and/or conceived via the Fenway Health AI Program. In addition to insemination, the AI Program offers social and emotional support for LGBTQIA+ parents and prospective parents.
Violence Recovery Program
More than 400 survivors of domestic violence, sexual assault, hate violence and police misconduct were helped by the Violence Recovery Program (VRP) at Fenway Health. The VRP offers individual and group counseling to LGBTQIA+ survivors. VRP staff also conducts outreach and educational sessions to community groups and professional groups about LGBTQIA+ violence issues on a local, state and national level, including providing technical assistance and training to the Boston Police Department, District Attorney’s offices, hospitals, health centers, social service agencies and community and school groups.
The National LGBTQIA+ Health Education Center provides educational programs, training, technical assistance, resources, and consultation to health care organizations with the goal of optimizing quality, cost-effective health care for lesbian, gay, bisexual, transgender, queer, intersex, asexual and all sexual and gender minority (LGBTQIA+) people. In FY2023, faculty from the Education Center presented at conferences and facilitated trainings and webinars, educating health care professionals on a range of topics from achieving health equity to care for transgender and gender-diverse patients, to care for LGBTQIA+ older adults, to affirming care for intersex patients.

From October 27-29, 2023, we hosted the ninth annual Advancing Excellence in Transgender Health Conference at the Colonnade Hotel in Boston. It marked the first in-person gathering of the conference since the onset of the COVID-19 pandemic. Participants heard from experts skilled in the provision of culturally responsive and confident gender-affirming health care that is grounded in research, evidence, and best clinical practices. Schuyler Bailar, the first transgender athlete to compete in any sport on an NCAA D1 men’s team, and the only to have competed for all four years, delivered the conference’s keynote speech.
From March 24-26, 2023, we hosted Advancing Excellence in Sexual and Gender Minority Health Education, a national CME course developed in response to the high volume of queries from clinicians and health care staff seeking to learn about providing high-quality care for patients who identify as a sexual or gender minorities. The conference was organized for whole health care teams to learn about providing competent, confident and affirmative health care, grounded in research, evidence, and best clinical practices. Conference topics included Achieving Health Equity: Terminology, Demographics, Stigmas and Disparities; Creating a Welcoming and Inclusive Environment; Behavioral Health Integration; Trauma Informed Care; Primary Care and Preventative Health Needs of LGBTQIA+ Patients; LGBTQIA+ Youth and Adolescents; LGBTQIA+ Older Adults; Reproductive Options for LGBTQIA+ People; HIV/STI Treatment and Prevention for LGBTQIA+ People; and Collecting Sexual Orientation and Gender Identity Data.
In 2021, the Division of Education and Training launched a new four-year initiative funded by HRSA HIV/AIDS Bureau titled The Innovative Intervention Strategies Coordinating Center for Technical Assistance (2iS CCTA). This project aims to identify and provide support for the implementation of evidence-informed interventions to identify existing innovative intervention strategies, pilot test the intervention strategies at up to 20 Ryan White HIV/AIDS Program (RWHAP)-funded sites, provide technical assistance (TA) to support implementation at the sites, and develop accessible dissemination products to promote the replication and scale-up of the intervention strategies in HIV service delivery organizations nationally. The four intervention focus areas are: incarceration experience, LGBTQ+ youth, substance use disorder and telehealth services.
In FY23, the LGBTQIA+ Aging Project continued convening its twice weekly Zoom Drop-In group to reduce social isolation and loneliness. Total participant registration increased from 104 in FY22 to 150 in FY 23, with 47 people attending in FY 23, and an average group size of 15 participants. The group met on both Thanksgiving and Christmas to reduce holiday isolation, with 18 people attending on Thanksgiving and 15 on Christmas. Most of the holiday participants said that they wouldn’t have had contact with other people on these holidays if not for these groups. Many newer participants are from outside of Massachusetts, indicating limited online resources for LGBTQIA+ older adults elsewhere in the country.
The twice monthly Virtual Visits for transgender and gender expansive older adults also continued via Zoom. With a total of 105 people registered, these sessions averaged 12 participants with open conversations and mutual aid to reduce social isolation, share resources and experiences.
The Aging Project hosted four bereavement groups for LGBTQIA+ people of all ages who have lost someone important to them; all participants valued a LGBTQIA+ safe space to address loss. With the return of Boston’s Pride parade (first since 2019) The Aging Project coordinated LGBTQ Senior Pride’s participation in Boston Pride 4 the People’s Pride Parade, with a record number of 80+ LGBTQ older adults registering to ride our trolleys and celebrate Pride again.
The LGBTQIA+ Aging Project continued its leadership on the Massachusetts LGBT Aging Commission, chairing meetings, work groups and engaging members to achieve identified goals.
The LGBTQIA+ Aging Project delivered a webinar series on implementation of LGBTQIA+ inclusive practices for elder service organizations, provided technical assistance to individual organizations and consulted with organizations throughout the country, interested in launching LGBTQIA+ inclusive programming. Throughout FY23, the LGBTQIA+ Aging Project conducted 30 trainings, workshops, and webinars for a total of 1948 participants. Staff participated in three national conferences, and two local ones. The LGBTQIA+ Aging Project also co-hosted the 13th annual LGBTQ+ Elders in an Ever-Changing World Conference, New England’s premier LGBTQ Aging Conference, which continues virtually and engaged 371 participants representing 27 states.
During 2023, The Fenway Institute continued groundbreaking research into biomedical and behavioral health interventions designed to enhance the health and well-being of LGBTQIA+ people and people living with HIV.

Adapt eSense, a sexual function intervention originally developed for cisgender women with sexual arousal and desire concerns, for transwomen at any stage of the gender affirmation process
The AMA Foundation-sponsored Harvard Medical School (HMS)-Fenway Health LGBTQIA+ Health Fellowship Program equips primary care residency graduates with the skills needed to serve as influential physician leaders in the field of LGBTQIA+ health.
Applications will open in spring 2024 for two Family Medicine, Internal Medicine, or Med-Peds physicians to join our 12-month (July 1, 2025-June 30, 2026) academic and career development fellowship in LGBTQIA+ focused primary care.
The Fenway Institute shared our learnings and discoveries in countless boards, committees, conferences, publications, and trainings around the world this year, profoundly impacting the field for the benefit of our community. Notable examples include:
Dr. Madina Agenor, The Fenway Institute Faculty member, was awarded the 2023 NIH Sexual & Gender Minority (SGM) Early-Stage Investigator Award, recognizing Dr. Agenor’s substantial and outstanding research contributions in areas related to SGM health.
Dr. Abby Batchelder, The Fenway Institute Faculty member, was promoted to Associate Professor of Psychology at Harvard Medical School, based upon her significant original scholarly and scientific contributions to the field.
Andrew Camarena, Community Engagement and Recruitment Manager earned his Masters of Public Health degree from Boston University.
Dr. Alex Keuroghlian was awarded the HMS Scholarly Engagement Excellence in Student Mentoring Award for 2023, recognizing Dr. Keuroghlian’s excellence in mentoring HMS students in their scholarly projects, and as a role model and resource for medical students working towards residency.
Dr. Ken Mayer, Medical Research Director and TFI Co-Chair, was awarded the Michael A. Tye Leadership Award, recognizing exemplary leadership on behalf of Fenway’s mission and the communities it serves.
Dr. Jenny Potter, TFI Co-Chair, was awarded the 2023 AAMC Group on Women in Medicine and Science Leadership Award for an Individual (Established), recognizing Dr. Potter’s outstanding contributions to the advancement of women in medicine and science.
Fenway Health switched to Epic as our medical record software in December of 2022 and The Fenway Institute’s Data and Health Informatics team were key leaders in that transition.
Fenway is part of a group of 12 area community health centers known as the Community Technology Collaborative (CTC) that all switched to Epic, a best-in-class confidential, patient-centered platform that allows us to have a single fully integrated system to better manage patient care.
As with any implementation, we had our hurdles and we continue to work with CTC to update our workflows to serve our community best and perform better for our research and state-required UDS reporting. Overall, Fenway staff like the charting efficiency in Epic, the improved times for prescription refills and prior authorizations, and communication via the Epic InBasket messaging system that allows teams to communicate more easily with one another as well as with the patient via their Epic MyChart portal. Since Epic is one of the most common EMRs at medical facilities across the US, our patient data can integrate with other systems, allowing us to view patient encounters at other facilities using Epic. This is crucial for closing care gaps and meeting quality care needs.
By publishing policy briefs, submitting public comment and testimony, writing op/eds, participating in media interviews, and joining coalitions in partnership with other institutions, Fenway Health staff and affiliated researchers and scientists engage in broad efforts to advance and disseminate LGBTQIA+ and HIV-related health policy research.
During fiscal year 2023, Fenway programs, publications, staff experts, and events were featured in more than 2,000 news stories on issues related to LGBTQIA+ health, access, and equity.

On May 20th, 2023, 650 of Fenway Health’s supporters and community leaders gathered at the Omni Seaport to celebrate and contribute to the mission of Fenway Health. The event was praised for its unique style and flair, which included local dance troupe Ladies of Drama, a fabulous afterparty and fashion worthy of the Red Carpet. The highlights of the program included great auction items including Tiffani Faison’s hosted dinner party, a show stopping story from Youth on Fire team and generous support from everyone in the room during the challenge. Congratulations go to Boston Pride for the People (BP4TP) and the Hale family as each received an award for their support of the community in two very different ways. BP4TP brought Pride back to Boston and the Hale family provided millions last year to philanthropic community causes, including Fenway Health.

The annual ride from Boston to Provincetown is so much more than a fun and exciting event. The October 22, 2023 AIDS Walk was set up to be a wonderful event, but the remnants of Hurricane Ian had other ideas. The severe wind coming off the ocean made it unsafe to walk along the beach, and all the tents involved in the wellness festival became a hazard. Although the event had to be cancelled, the positive impact of the fundraising efforts helped deliver care, treatment and research for people impacted by HIV/AIDS. Thanks so much to our dedicated volunteers who lead fundraising teams and who, along with our corporate sponsors, collectively raised more than $250,000.

The Young Leaders Council is an initiative of Fenway Health that aims to empower emerging LGBTQIA+ leaders and allies to shape their community’s future. As a worldwide leader in LGBTQIA+ healthcare, we have a role and a responsibility to every patient. We have an obligation to address any systemic or cultural obstacle that prevents top quality care. Given our position in the LGBTQIA+ community locally and nationally, as well as our voice in the healthcare sector, we believe we can be most impactful by focusing our attention on these 5 broad areas of urgent concern.
We seek to address these significant societal and economic health concerns through world-class patient-centric care, public health initiatives, research, education, and advocacy. These healthcare concerns disproportionately impact LGBTQIA+ people and other marginalized groups.
To that end, a small number of donors have dedicated their contributions to the Future of Fenway. These significant and multi year commitments enable Fenway to realize its potential today and for generations to come.
